Thanks for sharing the training feedback format. It is even more important to take feedback from the senior employee to assess the effectiveness of the training provided. The main purpose of training is to enhance the employee's capabilities. Obtaining feedback from the employee about the training may seem like a formality and may not fully serve the purpose of training.
Therefore, I humbly suggest modifying the form appropriately so that the effectiveness of the training can be evaluated by a senior person.
