The license is mandatory in your case as you fall under the "Factory Definition" according to the provisions of the Factories Act 1948.
You need to look for the relevant form under the Factories Rules applicable for your state. Use the format by adding annexures to the form as required, and attach a manufacturing process flow chart, a list of board of directors, as well as remit the fee. The fee is mostly based on the horsepower used and the number of manpower employed.
Most state governments have now made it simple and online. You may visit the website of the Department of Directorate of Industrial Safety and Health for more information and upload your application.
