Hello Laxmi,
Download the "Joint Declaration by the Member & the Employer" form which is available on the EPF website. Mention the details pertaining to the specific corrections that are required (it could be in the member's name, Father's/Husband's name, DOB, DOJ & DOL). If the correction is with regard to the member's name, Father's name, or DOB, you would need to attach a self-attested PAN Card (member) along with this duly filled and signed form. If the correction is with regard to the DOJ or DOL, you need to attach Form 5 & Form 10 along with the duly filled and signed "Joint Declaration" form. Once these submissions are done, you can expect the corrections to be done within 15 days. Please remember to keep the acknowledgment copy with you for all these submissions. If corrections are not done within 15 days, you can register your grievance on the EPF website itself.
