I worked with a company in Gurgaon (Haryana) for 3 and a half months (from 5th February 2013 to 20th May 2013). They deducted my ESI every month as my salary was Rs. 14,500/-. However, neither did I get my ESI card nor was I asked to fill any form by my company.

Guidance Needed for ESI Submission Check

Can somebody guide me on where I can check if my ESI was submitted to the concerned department or not? In case it was not submitted, then I will claim the deducted ESI amount from my company.

For your question, my suggestion is to ask the ESI number from your company's HR personnel, which they remitted the previous month. If you have the IP number, simply approach your local ESI office to inquire about your balance details. If they have made payments, you will be able to ascertain the amount paid and verify your contribution.

Kindly approach your company's HR department to inquire about your ESI number. You can also inquire about your ESI Card. After one month of joining, you will need to fill out Form 1, and subsequently, you will be provided with a temporary ESI card. Within 1-3 months, you can expect to receive your smart ESI Card. Do not hesitate to communicate with your HR team regarding these matters.

Yes, you can find the information on ESIC: Chinta se Mukti. Click on IP/EMPLOYEE/STAFF Login and use your IP number.
