My organization used to hire employees from third parties and there are only 12-15 employees on payroll and others are third-party employees.
I have worked for one year. There is no HR. I asked for my salary slip in the account department, and they informed me that there is no specific format for it.

Now, did you understand why I said to take a salary certificate from your employer? This is because in your salary slip, they'll have to show the PF and ESI deductions. Therefore, your employer has not registered you for PF, and instead, they were paying you by cheque.
Do not wish to prepare your own salary statement; always request it from the employer. If you persist, here I can suggest that you take a basic salary of more than 40%, which shows that you would not be applicable for ESI contribution, and for PF, leave it blank.
