Dear All, I need Form 5A for getting approval from the Inspector of Factories to work on NATIONAL HOLIDAYS. I could not find this form in the net. Plz any one give me the FORM 5A format.

Various forms are provided by the Factory Rules (Central & State). Therefore, first decide which of the State or Central Factory Rules are applicable in your case, and then go through those Rules to obtain Form 5A if you are unable to find it online.

My weakness is that I am not a good speaker; I am afraid to speak in front of a group of people, and I am not good at drafting as well.

I agree with Mr. Khola that you must specify in which state your factory is operating. Additionally, I would like to inform you that you can obtain Form 5A from the website of the Ministry of Labour and Employment. The link is labour.nic.in/.
