Like Binu said, there is no standard format for resignation. Resignation is a personal decision of an employee, and how he gives expression to this decision is, again, his personal choice of words and expression.
I can assure you that framing a resignation letter is a matter of skill. In my long work experience, I have helped several people to draft their letters of resignation. A wrongly worded resignation letter can easily lead to sure disaster.

Dear Kuldeep,
In our company, there is no specific format for the resignation form. If any employee wishes to resign, they should write a letter in their own words and send it to our HR department. The HR team will then provide the Full & Final form along with the Exit Form.

Resignation should be short and addressed to HR. The date must be mentioned in the letter, and the employee may request their relieving date. The resigned employee should mention the reason for leaving and also highlight their experience with the organization.
