I recommend you writing the memo on your own. Some senior member like will correct it. If you ask for ready made draft then it would tantamount to spoon feeding and it is insalubrious to your career. Some time you need to develop business writing skills. If you keep on deferring it then when will you build these?
In your draft include specific instances of negligence. Do not use the words like negligence or carelessness but write the facts and compare these with standard norms.
By the way, have given him proper Job Description of his position? Along with memo, list out his complete duties. Segregate his activities in daily, weekly, monthly, quarterly, half-yearly and yearly schedule and give it him. If you provide him list of his duties, it will become easier for you to point out his future lapses.
