Is it mandatory to fill out the previous employment details at (10, 11 & 12) if the response is YES to either or both of (8) and (9) in the declaration PF form 11?
Previously, I worked at two companies. Presently, I am in my third company. My first company deducted PF for only one month and allocated a PF number. After that, I left that job. I still have the PF number from the first company. The second company also deducted PF. I withdrew the EPF & EPS amounts upon leaving.
Now, in my current third company, the employer is requesting to fill out and provide a declaration in PF form 11 (New). In this form, even though I withdrew the EPF & EPS amounts from my second company, is it necessary to fill in the previous employment details at (10, 11 & 12) if the response is YES to either or both of (8) and (9) in the declaration PF form 11? Otherwise, should I just mention my first company's EPF number, considering they deducted PF for only one month?
Kindly clarify my doubts. For quick reference, I have attached the Declaration PF form 11 (New). Please find the attachment.

EPF Form 11 Submission Guidelines
If you have closed the second company's PF account, it will not be active. Therefore, please do not include the details of that account in Form 11 submitted to your current employer. Since you have not withdrawn funds from the first PF account, you can provide details of that account in Form 11.
The Form 11 you submitted is now irrelevant. The EPFO has introduced a new Form 11 to replace the previous one, and you are required to complete and maintain the new form. It is uncertain when the EPFO will discontinue this form, as they have made it a routine practice to introduce new requirements such as KYCs, UAN, etc., thereby increasing the workload for HR personnel. They seem to believe that HR professionals are solely responsible for EPF-related tasks and have no other responsibilities within HRD.
Please locate the updated Form 11. You are only required to print out the third page.
