Below are the documents required to submit to your last employer:
• Form 19 (for EPF withdrawal)
• Form 10c (for EPS withdrawal)
• A blank canceled check (this is required to verify the accuracy of the MICR Code Number). The check should be in your name as a single account holder and not a joint account check.
