Quiet a well thought out questionnaire.
But i noticed that it is conducted by an internal Analyst, would it be possible if you could post a questionnaire conducted by an external Analyst for a health providing organization relating to the effectiveness of Management information system.

Excellent questionaire! Well thought. I have a few questions need your clarification:
1. How would this form be circulated? To the HOD only or to every employee?
2. Base on your experience - how long will it take for you to receive all responses?
3. From the questionaires received - how would you interpret the data for?
4. If you can briefly let us know the process would appreciate it.
The reason of this questions because in my organization we have 4000 and growing employees. Therefore, i need to think carefully as how to get responses on time and accurate information.

Training needs can be identified through questionnaire. True. BUT THIS NOT THE ONLY METHOD. It may be popular. But I strongly recommend OBSERVATION as a method to identify training needs. You can also use unstructured interview for this purpose.

Instead of asking for a completed project/questionnaire, please search CiteHr where some projects are attached. Then do a literature survey and formulate your aim and objectives for the research (in consultation with the host organisation). You also need a conceptual framework for that. Decide what hypotheses you want to test.
If you have not done any of the above essential preparation for conducting a project, I would advise you to please do so and then pose a precise question.
