I have a PF number. My company's head office was in Mumbai, but the company has been closed since 2005. I worked there for one year. Now, I want to withdraw my PF from that company. However, I don't know where to send my PF form or what formalities I need to complete. I don't have any address or contact numbers to inquire. In that case, please fill out your PF forms and meet the PRO for the same. You should also meet the Assistant PF Commissioner. He will help you in this regard. There are other formalities as well. It's better to call me so that I can brief you about the same.

In form 1995, it is stated that if, for any reason, the claimant is unable to have the application attested by their ex-employer, they may submit the claim to the PF office after having it attested by one of the following authorized officials:
• Manager of the bank where the member has an account.
• Head of an educational institution.
• A gazetted officer.
• The magistrate.
• Employees Provident Fund or Regional Committee and others.
