How to design questionnaire to measure Competency(Knowledge, Skill and Ability) and the effect on performance

Dear Ramesh,

I use this opportunity to implore that we should give enough info to bloggers at the outset, for them to help us effectively. We need to give the following information:

What do we do (whether we are students, etc?) and where (in what type and size of industry)? For example, see how Jayeshm has posed the problem at Goal Setting

Why are we seeking the information? For example, is it for answering an assignment question, or apply it at work? By when do we want the info. (That's how urgent is it)?

Another important aspect is we should try and find information First (say search the web) and then seek Help, as one blogger keeps saying in his signature.

Good Suggestions to all to get the right answer here at citeHR. Infact it would be good for the bloggers too to understand the question before giving a good precise answer.
Though there are some fresher/new or you can say person with less writing capability to put their exact questions here but at least they can give the basic information about the query as you suggested.
