One of our employees has passed away due to COVID-19 while in service. He is survived by his wife, son, and daughter, with both children aged 9 and 6 years.

Claiming Provident Fund Benefits

The family is seeking clarification on the procedure for claiming benefits from the Provident Fund (PF). Could someone please confirm if a single COMPOSITE CLAIM FORM (CCF) is sufficient to claim all the benefits, including PF, Pension, and EDLI, or if separate submission of forms 20, 10D, and 5IF is required?

Entitlement to Ex Gratia or Bonus

Additionally, I would appreciate it if someone could clarify whether the family is entitled to any Ex gratia or a bonus of 1.50 Lakhs.

Upon the death of the employee, the nominee needs to make a claim for the PF, pension withdrawal, and EDLI in the composite claim form instead of filing three separate forms. Additionally, the bonus amount is also paid to the claimant.
