Generally, in most companies, leave applications are standardized and made available to employees in a printed format. You may also apply for leave online, but that format is also standardized. Therefore, you may check with your HR or Admin department and ask for the ready-made leave application. There is no need to write a personal application for a routine activity like asking for leave.
