Dear Seniors,

I want to know if Form No. 1 (ESIC), 2 (PF), and 11 (PF declaration form) for employees are mandatory to submit in the local PF office. Nowadays, employers are conducting online registrations for new employees, and all processes have shifted from manual to online. Therefore, I am curious if these forms are still compulsory for submission at the local PF/ESIC office.

Even though systems are getting online, there is no intimation or instruction from either PF or ESIC for non-submission of any of the above-mentioned forms. To avoid any problems in the future, it is suggested to keep submitting all those forms on time to the respective authorities until no intimation comes from them.

    Please note ESI Form #1 - online Temporary ID Card (The question of submitting to ESI does not arise).

    Please note EPF Form #11 (New) Declaration Form to be obtained from all employees joining an establishment. Duly completed and signed Form 11 to be retained by the employer for the sake of record.

    EPF Form No. 2 - Nomination Form - Consequent to the introduction of online from March 2012, it is not necessary to obtain the same. Even if you obtain, file it in the personal file for future reference.
