I am working in the field of HIV/AIDS and I want to conduct a training impact assessment of employees who are working in the field. Various cadres are involved in this program such as Program Manager, Counsellor, Monitoring & Evaluation Officer, Outreach Worker, and Peer Educator. Please provide me with some ideas for the questionnaire to collect data.

Dear Sukhendar,
You can design the assessment questionnaire containing the questions on skills of counseling, conveying the message of being HIV positive to clients and family members, and extra efforts required in case when a female is found to be HIV+.
I have quoted all these because I was a supervisor for monitoring the skills and performance of counselors at PHCs and CHCs.
