What forms should the injured employee carry to the ESI hospital? Also, when the employee is ready to return to duty after complete recovery, what documents should they submit to the employer? Will the employee receive a salary from ESI, and if so, for how many days?

Ensuring ESI Compliance for Workplace Injuries
Firstly, ensure that all employees working under you and covered under the ESI Act, 1948 have ESI Identity Cards after obtaining their Insurance Numbers from the appropriate Branch Office of ESIC.
As soon as an employee/Insured Person suffers an accident related to the work of the factory/establishment, submit the accident report to the appropriate Branch Office of ESIC and other State authorities.
It is the responsibility of the principal employer to provide first aid to the injured employee.
The injured insured person must carry their identity card issued by ESIC.
The employer can obtain a copy of the medical certificate issued by the appropriate dispensary/hospital.
If there are any questions regarding the eligibility of the concerned insured person, it is advisable to immediately contact the Branch Office of ESIC with relevant records so that the injured person can receive medical treatment.
Under the ESI Act, the benefits paid in cash are not termed as "salary." If the case is accepted as an employment injury, Temporary Disablement Benefit (TDB) will be paid to the insured person until the date they are certified as incapacitated due to the accident. The rate of Temporary Disablement Benefit is approximately 90% of the standard benefit rate in the relevant contribution period (Rule 57 of ESI (Central) Rules, 1952).

I just want to add a point. In case of an emergency, you or his immediate supervisor must move him to any ESI Dispensary. If ESI Medical is not available, then the patient must be taken to any private hospital having a minimum 16-bed facility. Within 24 hours, the report must be sent to ESI officials, and you have to register there by obtaining a letter from the doctor who is taking care of the patient.
