Procedure for Changing Company Name in ESIC Registration
For a change of name of the company/factory, you can send the intimation to the appropriate Sub/Regional Office of ESIC, as well as the Branch Office Manager and the Social Security Officer of your area. Intimate them about the new name and principal employers of the company/firm, as well as the date of such change. The said Sub/Regional Office is expected to note the same in their record and send you confirmation as well.
However, the liability of arrears/dues, if any, will be on both the old and the new company, as well as on the principal employers of the same. Kindly refer to Section 93A of the Employees' State Insurance Act, 1948.

Thank you, Mr. Harsh, for the reply. I want to know if I should send an intimation letter only or include some documents regarding the changes in the company name. Alternatively, is there a form that needs to be filled out for the name change process of the company?

There is no prescribed form for this process. You can send a simple letter to notify about the change of the name of the company or firm. If the change of name is based on a change in the partnership, you can send a copy of the revised partnership agreement. In the case of a company, you can send a copy of the order of such a change from the Registrar of Companies.
If the ESIC offices are in the same location as your establishment, you can send the intimation through your own staff, but try to get acknowledgment from the said offices.
