1. How the training was conducted
2. Questions on Trainers (trainer's feedback, trainer's impact on participants)
3. Training effectiveness
4. Training needs identified (process & weightage, employees' feedback)
5. Employees' feedback on training sessions
6. Employees' feedback on bringing about changes in training and development activities
