I had worked for Brightpoint India Pvt Ltd. in Lower Parle, Mumbai for 2 years. I left the organization 2 years ago. I applied for PF, but as of now, I have not received it. The branch in Mumbai has since closed, and I am unable to contact the HR department for my PF.

Hi Preeti,
I am attaching herewith the PF Withdrawal Form No. 19. If the Employer is not available, you have the option to get the form attested/signed by any of the following: a) Magistrate, b) Gazetted Officer, c) Post/Sub Post Master, d) President of the village union, e) President of the Village Panchayat where there is no Union Board, f) Chairman/Secretary/Member of the Municipal/District Local Board, g) Member of Parliament, Legislative Assembly, h) Member of Central Board of Trustee/Regional Committee, Employees' Provident Fund, i) Head of any recognized educational institution, j) any authorized official, as may be approved by the Commissioner of PF.
Fill in the application form attached, get it attested by any one of the above, and submit it to the PF Commissioner's office, then obtain an acknowledgment.
Provide your Bank Name, Bank Account Number in the application, and the bank's address.
You will receive your PF contribution amount deposited in the specified bank account, and you will be notified. If you do not receive the amount even after one month, you can visit the Enquiry office at the PF Commissioner's office to inquire about the reasons for non-disbursement.
Everything regarding the withdrawal of PF amounts is clearly specified in the attached documents.

Though you worked in Mumbai, your company PF accounts were found maintained in Delhi PF office (by seeing the account number given by you - assume your PF number as: TN/7756/5665 wherein TN stands for Tamil Nadu, 7756 denotes your establishment code number, and 5665 denotes your number). So you can approach your head office HR department for withdrawal. Otherwise, you can do the following:
Procedure to withdraw the PF amount:
Step 1: You can contact the local PF office and collect Form 19 and 10C or download withdrawal forms in Form 19 and 10C, NSSN FORM from the website [EPFO](http://www.epfindia.nic.in).
Step 2: You can fill up all the details about yourself and fill the bank account column with your savings bank account.
Step 3: Open a savings bank account preferably with a nationalized bank - mention the account number in the forms 19 and 10C - also attach a xerox copy of the first page of the passbook (depicting the bank account number, branch location, etc.).
Step 4: You send the complete set of forms with enclosures to the previous employer for forwarding the forms to the PF office.
Step 5: You can obtain attestation in the column of attestation of employer/authorized person (if you want to send the claim straight to the PF office) from the bank manager with which you are having a savings bank account.
Step 6: You can send the set of forms to the PF office in which your previous employer has accounts - by registered post.
YOU WILL GET YOUR MONEY INTO YOUR BANK ACCOUNT WITHIN 45 DAYS.

You can take the following steps: properly fill out your Form 10C & 19 and speak to the nearest PF commissioner about the issue; he will surely assist you in this matter.
