Yes, you can visit the ESI website and download Form No. 1, i.e., the declaration. There is no form for a permanent card. Once the employee is allotted an ESI number along with a temporary identification certificate (which is part of declaration form no. 1), it is valid for 13 months from the date of appointment.
Permanent Card: The ESI local office will issue a computerized card to the member from the date of appointment.
