As Mr.Dilip said right accident investigation techniques vary from Place to place, Company to company, Even it will vary from you and me but the concept of investigation is same.,,

Dear Raghu,
You are my type of man - collecting different useful material from different sources and keeping them in a library. i do the same as well. You never know when they become useful.
I am attaching 2 files - one a procedure I wrote for an upstream company & the other a training presentation I gave to a shipping company where the TRIPOD BETA methodology is used.

Dear Mr. Pillai,
MISCTRIPOD ppt is excellent.
In slide nos 20 and 21 you have used shortform as TRC in the triangle comprising Empathy, Culture, Motivation, At Risk Behavior, Near misses, TRC, LTI and F.
Kindly explain what TRC Represents for.
Similarly in slide no 69 you have mentioned PAER what does these letters mean.
I am working in Enginering company and these terminologies are new to me.
I liked the entire presentation, I feel some consepts of FTA and ETA are used.
The example quoted about burn injury makes it easy to understand.
