Dear Sir,
It is quite interesting that the subject has aroused. The simplest method is given by the management systems like OHSAS 18001 and BS8800.
These management systems asks us do Hazard identification of our process activities which simple and vital.
If anything goes wrong in our quality activity then the product quality is affected.
If anything goes wrong in our activity connected with environment it has an impact in our environment.
Similarly if anything goes wrong or we not following the safety prosecutions required then it will affect the employee working the work place.
So let us first do hazard identification and then take risk control measures.
This is the minimum requirement and simple way instead of doing accident investigation and take corrective measures.

Dear Ms.hansa,
I am just seeking some pleasure by participating in this interesting forum. What I gained through years of working in safety I am trying to impart now to youngsters. I am really afraid for youngsters being shy to ask questions. The foolish pride makes them shy and keeps them ignorant.
We are not here to worship what others have preached. We are here to question, get fully convinced and develop our capability to convince others on safety matters.
For writing in this forum we have constrains. However we should not relax.
Let us continue with our little contribution for the sake of this divine profession.

Dear Sir
One of the reason for less participation I feel is that if one post (May be New Commer or existing member) any quiries in the forum and not getting a satisfactory answer, he get desperate.
This can be one of the reason for people not coming forward. So we must try to answer every question related to Safety for getting more participation.
