If employee rejoin the company with diff pf no. What is the procedure - should esi no will be alloted again? What is the procedure for temporary card and permanent card. please send me forms too

Once a employee having ESI No same should be used at the time of Rejoining of the employee. Because of Smart Card phenomena now there is now no Temporary /Permanent Card.

In ESI there is no need of re registration, if an employee re enters his employment after any gap. The re entry date has to be notified through making entry in the remarks column of the ESI return in Form-5.The old card may be used by the employee concerned.Present smart card may be issued by the ESIC too.

I agree with ur first point only...........Anyway I will educate our members about this :
* Try to think about the problem practically...Y because, as of now every ESI branch Office has its own BLOCK Nos. in alloting the Insurance Nos. to the employees working in the factories/estt. working in its jurisdiction......
* For certain diseases for making the benefit amounts, we have to see his insurable employment for the last TWO YEARS, hence while taking steps in respect of the newly joining employee, who has already covered under ESI in the previous Unit the following steps to be followed by all the HR Personnels:
1. When the new employee reveals that he has already covered under ESI and have the particulars of ESI No., then u have to fill up a form called ESIC-53 and submit to the Branch Office concerned in which jurisdiction the new factory/estt. falls... then the Branch Office will get his previous records, such as 'Declaration Form/Ledger Sheet/Returns Particulars' from the previous ESI branch office in which jurisdiction the previous factory/estt. is attached;
2. After receiving the previous Declaration form, they will allot their running ESI Block no. in the place of the old no., the same no. to be reflected against his name in the concerned returns; though he was alloted new no., "by virtue of having his old particulars, he will treated as under continuous insurable employment and he was entitled for all cash benefits, as applicable".
3. If the previous factory & new factory falls in the same Branch Office jurisdiction, he has to submit ESIC-53 with particulars of change of employer particulars, in which case he has to use his previous no. only;
AFTER COMPUTERIZATION:
01. If earlier he was provided with Smart Card, then there is no problem, after joining the new co. in the Branch Office, simply in the software they will change the employer particulars only, since AFTER COMPUTERISATION NO NEED TO CHANGE UR ESI NO., AS UR CARD PARTICUALRS ARE PERMANENT IN NATURE"
02. Though the Computerisation is on, its better to request for issue the Temporary Cards, since after enrolling the Photo Camps, it will take min. one month to get ur cards, till such time, whats the fate of the Insured Person, as far as medical facility is concerned.........HENCE ITS ALWAYS BETTER TO HAVE TEMPORARY CARD.........till u get the SMART CARD.

thanks ,so much,but many of employee in my company attended camp for esi biometric cards, its almost 4-5 months,still they didnt get their esi card,what to do?

I already told u MINIMUM 1 month, don't worry about it, since being at the initial stages some delay may occurs, For getting the cards or confirmation of receipt of cards, u can keep in touch with the concerned Branch Managers.....

one of our employee already have permanent IP number of ESIC of previous employer now he appoint in our company so i wanted to know weather he can use his previous permanent number if yes then what is the procedure for getting same permanent IP number

I want to educate you about some practical issues after computerization.......thereby all your doubts will be clarified:
Here I am discussing various instances that HR people will come across after introduction of smart cards in ESIC:
i. Your new employee might been having the old ESI Card (i.e. not Pehchan/Smart Card) & not yet attended the Pehchan Camp & now he joins in your company: In such cases, you go to insert IP details of your portal, enter his ESI No. particulars and give the required inputs, thereby he will be included in your list of employees. Thereafter you update his complete family and other particulars by opting the link of "Update IP Particulars" and advice him to attend the Smart Card or Pehchan Card camp immeditely.. (NOTE: ONCE YOU KNOW THAT THE EMPLOYEES HAS ALREADY ATTENDED FOR PHOTO CAMP IN HIS PREVIOUS EMPLOYMENT...DON'T GENERATE THE NEW NO. FOR SUCH EMPLOYEE, AS IT LEADS TO DUPLICATION WHILE GETTING PRINT OF THE NEW CARD AND U HAVE FACE PROBLEMS FOR GETTING HIS EITHER OF THE CARD);
