Hi all, 'm working as a Fire/safety eqpt. mant. foreman pls help for the following ?? for filling the self appraisal form...
1. What are the major achievements in the past appraisal period???
2. Who are the \"coustomers and how well have u served your customers during the past appraisal period???
3. Are there areas of your performance in which you could improve?? no.... yes..., if yes please describe.

This is a self appraisal you need to look in to your annual performance and write down your achievements:

* Training

* Nil accidents - the precautions taken

* implementation of 100% personal protectice equipment utilization

* 100% compliance of work permit

* no adverse reports from PCB / Inspector of Factories wrt safety & Hazard

Customers:



* Internal like Production / maintenance and other related departments

* external - PCB / Factories Department

* your contribution towards educating the employees / training / conducting the programs like fire safety etc.,

Areas of Improvement:

* any area you feel need to be improved

this is on personal; front like undergoing on specific training program / upgrading your knowledge & skills

towards upgrading the skills of the employees / pending works like mock drills / fire fighting training / first aid training / 100% completion of MSDS / certification of equipment like chain blocks / lifts / hoists etc etc.

for completing this formailties you should have your Job description and your goal / KRA sheet then only you will be able to write down all the achievements wrt the goals otherwise it will be difficult for you because you don't have your target then how you can measure and confirm the achievements
