Is there any person or team who is responsible for regular maintenance and ensuring workplace safety? Please investigate. In that case, the responsible person should be held liable, and a warning letter can be issued to them.

In cases where the maintenance team is maintaining the checklist, they should be held liable.

It is also necessary to identify at the time of an accident whether the injured person knows how to use the equipment properly or has knowledge of basic precautions.

Mr. X,

This letter is to inform you that a serious complaint has been raised against you due to a recent accident where a scaffold board broke. The lack of proper fall protection and safety precautions are cited as reasons for this incident.

It has been observed by the management that your negligence in failing to rectify the defective board during maintenance of the scaffold and regular routine checks caused the accident. Moreover, questions have been raised about whether you ensured effective fall protection.

However, in view of your admission of the act and your verbal assurance that you will not allow such or similar instances in the future, you are hereby let off with a WARNING.

Additionally, your lack of preparedness for safety precautions indicates negligence in your work and will be recorded in your yearly performance evaluation. It is hoped that you will take advantage of the leniency shown to you and strictly adhere to your assurances.
