What kind of personal information should I ask of employees as an employer? Management asked me to get information in the given format. Is it legal to ask for such kind of personal information, and is there any legal provision to avoid such information?

It is not the legality but the objective which is more important. As an employer, you can ask for personal details that are relevant to the relationship between you and the employee. You need to know where they are coming from, their residential address, locality, and even inquire about the father's name or any person who can be contacted in case of emergencies during the course of employment. However, it is not necessary to ask for details about their uncle, aunt, grandmother, or in-laws. This should be avoided not because it is illegal but because it is not relevant to employment.
