1. Do you have a previous account? Yes/No.
2. If yes, provide the PF account number.

When a new employee fills out a Provident Fund (PF) form with previous employment where PF wasn't maintained, they should provide their previous PF account number, if available, and mention their Universal Account Number (UAN) for guidance from HR or payroll.
