You can directly approach the HR Department in your company for this case. There are two forms for the withdrawal of PF: Form No. 10C for the Pension Scheme and Form No. 19C for the Provident Fund. I have also attached these forms for your reference.

What Mr. Vineet stated is correct and applicable for claims made within one year of leaving the service. If the period exceeds more than a year, then you have to submit the following:
- PF Claim forms (F19 and F10c)
- An indemnity bond
- Copy of your PAN Card or Voter ID
- Bank passbook first page xerox
- Relieving letter issued by your previous employer mentioning your PF Number.
