You should go to your regional office of ESI and submit your Declaration form with your employees' names, fathers' names, ages, relations with the employees, and addresses. Also, fill out form-01 for registration. Additionally, you need to make a six-month advance payment through a challan.

- Sales & Purchase Bills
- Memorandum of Association (MOA)
- Shop License
- PAN Card of the Establishment
- Salary Register
- Phone Bill
- Electricity Bill
- List of Directors
