I am Shabana, a Senior HR Executive at an IT company in Pune. I am currently working on preparing appraisal forms for non-IT staff (Admin, Accounts, etc.). I am looking to create a form where both the appraiser's and appraisee's comments can be included.

First, prepare a detailed job description and analyze whether the candidate is effective and efficient. Next, for subjective criteria, use a ranking method (i.e., 1-5) and ask them to provide detailed instances if scores are extreme. Ask the candidate to fill it out, then discuss it with the boss, and finally, have both of them sit together to resolve any disagreements.
