I want to know how to write a letter to mnager about my Maternity leave details & need to get approval for those period?

Dear,

Considering that your unit is covered under The Maternity Benefit Act, 1961, and you are entitled to have the benefit of Maternity leave from the employer, you are hereby advised that you are to give notice under Section 6 of this Act to your employer in the prescribed format along with the medical certificate in the prescribed format as given in your State Maternity Benefit Rules where you are working.
