Could someone provide tips related to performance appraisal? I would be thankful if you could share some model appraisal forms to understand the parameters to be considered, techniques, etc.

I had to fight hard to get into this job, and I am willing to fight again to prove myself. Senior colleagues, please help me.

Please find attached a format for performance appraisal that I have created myself for our company. I have incorporated both qualitative and quantitative aspects that are common for all positions, enabling any employee to be appraised using this format.
