This situation needs careful handling.
First discuss with his team manager whether any problems-work side or personal has cropped up.
Secondly discreetly check up whether any medical problem has come up by discussing with his immediate work mates/team leader. All this is to be done in good way to encourage and pull him out of this issues.
Once information has been collected or there is no apparent reason, schedule a meeting with the person concerned and then talk it out in a congenial manner-with full details of his work, sudden dip in output/quality and seek to reconcile and suggestions to bring him back on line.
This is a sensitive task and should be left to mature and expereinced people as negative talk can push him further down to lesser productivity and loosing a good worker.Tact and freindly approach can help find out causes and mutual solution can be worked out.
A good worker is always an asset.
