Please help me out to draft a questionnaire to interview the cooks, waiters, F&B managers, chefs, etc.

Also, we are facing problems in store management in our restaurants. Kindly help me out in posting a good format that can be implemented in stores where there is a huge turnover in materials for our 10 outlets. I will be very thankful if you can lend me a helping hand.
