dear no supporting document is required to submit along with PF withdrawl orm.you are reqd to fill up for 10c and 19 and get it attested with employer. tks js malik

Hi Deepti Along with the forms 19 and 10C, You can give cancelled cheque for the proof of bank account, but not mandatory.

It all depends on how many years you have worked with the company. If it's below 9 1/2 years, then no document is required. Only Forms 19 and 10(C) need to be filled up and submitted, duly signed by the authorized signatory. However, if you have worked for more than 9 1/2 years, then supporting documents like proof of age of your nominees are required for the issuance of the Scheme certificate by the P.F. Authorities.
There is a separate column in both the forms that ask for the reason for separation from the company, but you don't need to write the reason for fund withdrawal in these forms unless, of course, you are taking an advance from your P.F. balance in Form 31.

Sometimes, if there is a delay in PF withdrawal, like 2 to 3 years or more, the PF office may ask for a reason for the delay to be addressed to the Regional PF Commissioner. They may also require the presentation of the bank account statement along with Form 19 and 10C. This recently happened with some of our colleagues.

EPF office has to give you a copy of Annexure K, which is the actual transfer paper. You need to preserve it to check with the next annual PF slip whether the transfer is accounted properly in the new account.
