If it is for an MBA degree, I am afraid your approach misses the objectives of the degree. I am sorry if you feel I am blunt in my response. I am guiding a few MBA students by email from the UK.

All of you doing projects, it will not be beneficial to the company or for you if you have no clarity with the objective of the project or the purpose of the project.
Getting a survey questionnaire, filling it, and interpreting it is not a project work. Without doing any research work, blankly asking for a questionnaire or asking, "How can I get information," is not why you are assigned to project work. Your project needs some life - get into it and ask the right questions. I am sure you will receive help.

You want to get the questionnaire filled. This was your question. I think there were many replies to your question but nothing relevant. The easiest way is: If you have a Gmail ID, go to Gmail documents. There you can make a form. Go to new, then click on form. You will get a form. Type your questions and alternatives there, and then you can send the link to your friends from whom you want to get it filled. You will get all the answers in a tabular form. That's the easiest way. Do try it!
