I had my name changed after marriage, due to which my PF transfer claim was rejected stating that the name is different. I raised a grievance with EPFO and received a reply stating that I need to fill out a joint declaration form and submit it to the EPFO office.
I have a query here: Is the joint declaration form I need to submit with my current employer or my previous one? My current employer already has the correct name, while my old name was with my previous employer. Additionally, should the seal and signature on the form be of my current employer or my previous one?

Hi , It needs to be done when you got married and with which employer you was at that time. Most important which employer is having your old name registered in there records.
