Hi, I am Aparna Vartak, an HR & Admin professional at Royal Translines Pvt. Ltd. I am organizing a company picnic to Goa for 2 days overnight in January 2018. I am looking for a format for employees' self-declaration for this outing. Please assist me with this.

You could have written a draft of the indemnity bond and uploaded it here for correction. Secondly, is the event for adults or for children? If the former, then why do you need the parents' signatures? Anyway, you may use the following draft:
+++++
Date: -
From: -
Mr/Ms ________ (name of the employee) Employee No _________
To,
The Managing Director ________ (name of your company) ________ (short address of your company)
Letter of Indemnity
Dear Sir,
1. I have come to know through a general circular that an outstation event has been arranged for the employees of our company at _______ (address of the picnic venue) from _____ (date) to ______ (date). I would like to participate in this event.
2. My participation is voluntary. I have read the guidelines issued about the event and will abide by them. Additionally, I declare that:
a) I am responsible for the safe custody of the jewelry pieces, cash, and valuables that I carry with me.
b) Though the event will be held off company premises, I will adhere to the code of conduct generally applicable to employees.
c) In case of any untoward incident, neither I nor my legitimate family members or nominees will hold my employer(s) responsible. No claims will be directed towards the company.
Thanking you.
Yours faithfully,
_________ (signature of the employee)
