Dear Sir, Regarding subject PF No. JH/RAN/0031594/000/000167, Universal Account Number 100721885266, Name - Kuldeep Ojha, Father's Name - Late Birju Ojha, Date of birth 09.07.1972. I am requesting an amendment for name correction and date of birth. I have already sent the documents to The Regional PF Commissioner (RO-Ranchi) via speed post on 16/01/2017. Enclosed are a Joint Declaration by the member and employer, Member passbook, Aadhaar Card, and a joint request letter (Member & employer). I am unsure if the above-mentioned amendments/corrections can be made due to my employment on a contract basis with Sri Guru Govind Singh Nursery, Hazaribagh. Thank you in advance.
