frist write accident book, TWO wtness is required, then log in to ESIC portal and submit the details of accident book there, take print out

Kindly visit the ESIC site and fill out the Accident Report Form 12 within 24 hours. Log into the website and navigate to the left side where you will find the Accident Report (Form 12) section. Enter your insurance number and complete all mandatory fields.
