Well, I appreciate your efforts. It is not possible to guide you or train you on English over this thread; however, I can give you some guidelines. English communication can be broken up into 1) spoken English and 2) written English.
Spoken English
First of all, continue reading English newspapers and also refer to a dictionary (keep one handy with you all the time; you can get pocket-size dictionaries in the market) for all the words you don't understand and try using them in a sentence. Make a habit of this. Watch English news channels. It will be equally impactful to listen to someone talk. Get cue cards from the market specially meant for vocabulary and start playing with them. Refer to all the words and use them in sentences. Do some internet research on grammar. You can also refer to online grammar training. Will you be able to join English learning classes?
Written English
Using proper punctuation is very important; they give different meanings if not used properly. This guidance is also available on the internet on how to improve written communication. Once you are proficient with spoken English, then writing in English shouldn't be a problem. Only writing etiquettes will require some focus.

At the outset, let me tell you that the problem you have shared is quite commonly encountered by many people. However, very few people take proactive steps, like you have done, to make an effort to tackle the problem by seeking help. In addition to the suggestions given above, I am sharing my thoughts on how you can work on getting quick results and improving your English language skills.
Understanding the Basics of English
To learn any language, you have to first understand the rules of grammar of that language. This will enable you to think, speak, and express yourself correctly in that language. To be an expert, you will have to expand your vocabulary and keep learning new words, phrases, idioms, proverbs, etc. Initially, you must focus on the grammar, and this may take some time. The English language has many exceptions to the normal rules, which makes it complex. The good part about English is that, unlike many other languages, all inanimate (non-living) things are of neutral gender, so you do not have to memorize the gender of various inanimate objects.
Key Attributes for Success
It is essential to remember that for anything you do in life, a clear goal/vision, adequate motivation/enthusiasm, hard work/discipline, and the ability to take risks/challenges are critical attributes. What you also require are the key attributes called 'discipline' and 'enthusiasm,' with which you can achieve anything you want. Your enthusiasm is the reason you wrote to us. The next test is to be disciplined and follow the suggestions given here.
Practical Suggestions for Improvement
1. My first suggestion is to read aloud as much as possible in English. A standard daily English newspaper and the Reader's Digest have very good reading material. If possible, record your voice and listen to it to identify the areas, sounds, words, and alphabets that give you trouble and on which you need to work. If possible, read aloud to a friend who can give you feedback based on which you can work on improving your pronunciation as well as articulation.
2. The mother tongue influence is a problem for many, considering that very few of us are native English speakers. To reduce that, make more English-speaking friends and make a determined effort to speak only in English, even if you keep forgetting words, tend to stammer, or your friends tend to laugh at you. This will require a lot of willpower, but believe me, you will soon improve. If someone corrects your pronunciation, don’t take offense; instead, be grateful that he/she is helping you correct yourself.
3. I would also suggest you try to learn at least 3-5 new words and look up their meanings in a dictionary. Then try and use these words in your daily work and communication to improve your vocabulary. Find synonyms for words that give you trouble and use the synonyms wherever possible.
4. If you still feel that professional help is needed, then you will have to attend English speaking classes.
5. Finally, if you can, do visit my blogs listed below regularly and try to learn the new words in them and also be inspired by them. Try to read the blog content aloud, and you can be sure it will be worthwhile since the blogs have a good response from the readers.

One of the common mistakes people make while writing in English is using 'I' (referring to oneself). No matter where in the sentence you use 'I' referring to yourself, it should always be in uppercase. For example: I have to leave. Can I join you? We are working on that, but I will still have to keep reporting.

You will automatically realize if you have used the wrong word, which you can rectify instantly, or you may become cautious about using that word incorrectly in the future. Also, don't be shy about using a mix of languages by incorporating some Hindi words if you have difficulty remembering the appropriate English word when speaking. These practices may help you improve continuously.

Practice makes perfect. Read newspapers, learn and use the new words that you can pick up in newspapers/magazines. Buy an Oxford dictionary and keep it with you for ready reference. Speak in English with your office friends to gain confidence. If you get a chance to speak in public, seize the opportunity and prepare well. Watch English news on TV. Keep trying out new words and do not get discouraged by any mistakes - learn from the mistake. Soon, your English will improve.
