If you have enrolled in ESI, you may obtain the ESI number from your employer, and you will receive a temporary ID card once you register. Your employer can provide you with the details of the dispensary nearest to you. Therefore, it is not necessary to visit your Head Office; instead, visiting the ESI dispensary closest to your location is mandatory to obtain approval for the permanent ESI card. You should go alone with your proof and speak with the ESI officer, who will guide you.

Lingabala, in that ID card you have to affix your Photography with your family members and get sign and seal from your employer so make sure it with your manager.
