I want to know the difference between the contribution period and the benefit period. If an employee joins the organization on 16 Apr 2012 and wants to avail ESI benefits, what information should be filled in Form 37 as asked by the ESI department?

Dear Naveen,
If an employee joins insurable employment, i.e., registered with ESI for the first time, they will be provided with a temporary card, now available online at the link of Print counterfoil, which is valid for up to 3 months from the date of entry of the employee. During this period, they are entitled to Primary and Secondary medical care at the Dispensary and ESI Hospital levels.
Regarding the ESIC-37 form, it is required in case the Dispensary/Hospital requests it. This form is a certificate given by an employer certifying either of two points: i) Whether the employee is still in insurable employment; ii) Whether they worked more than half the number of days or 78 days in the relevant contribution period.
Contribution Period and Benefit Period
There are two contribution periods in a year: 01.04 to 30.09 and 01.10 to 31.03. The relevant benefit periods are 01.07 to 31.12 and 01.01 to 30.06, respectively.
