Is your company a manufacturing unit or a commercial establishment? As per this classification, the leaves are governed either by the Factories Act or the Shops and Commercial Establishments Act. Once I know, I will provide more details. Normally, 21 days per calendar year are given.

In general, there are three kinds of leave:
1. Privilege Leave or Earned Leave
2. Medical Leave (sanctioned on medical grounds)
3. Extraordinary Leave (sanctioned under extraordinary circumstances and adjusted in the future period)
This type of leave is sanctioned only for employees with continuous service of more than 5 years, meaning it is for loyal employees only.
Leave Format
Below is a format for general purposes, but you can modify it according to your organization's regulations:
Name of the Organization
Address of the unit where leave records are maintained
1. Name of the Employee
2. Employee Code
3. Date of Joining the Service
4. Place of Posting
Details of leave availed during the year 2011

1 Opening Balance
Every organization has its own leave policy where the number and nature of leave are defined. So, I suggest you not worry about it. In case your company is small and does not have a formal policy but is financially stable, you can provide the following types of leave:
1. Casual Leave: 12 per year. This is for extreme emergencies and must be taken before or after a holiday. Those taking this leave must report the next day. More than one day is not to be sanctioned.
2. Privilege or Earned Leave: 2 per completed month. If not availed, this can be encashed, subject to a credit balance of 30 days at any given time.
3. Medical Leave: 20 per year, where half pay is provided during this period.
