How to Start an Appraisal System for a 50-Person Startup: Seeking Advice and Sample Forms

I would like to know how to implement an appraisal system for a startup company with a headcount of around 50, which hasn't had an appraisal system before. I'm planning to introduce it in the next financial cycle (April to March). What will be the starting process, and could you please also share an appraisal form for an ERP implementation company?
