Request for one day leave to go and collect my land title documents

Please i am working in an industry but want to request for one working day absence to go and collect my land title documents.
Am Finding it difficult to differentiate the right and wrong format and even the choice of word.

Generally in most of the companies, leave applications are standardised and these are made available to the employees in a printed format. You may apply for leave online also but that format is also standardised. Therefore, you may check with your HR or Admin department and ask for the ready-made leave application. There is no need to write personal application for a routine activity like asking for leave.
