How Should I Draft a Letter to Report a Damaged Office Phone to Higher Ups?

From my office, one of the staff members has a damaged mobile phone (screen break and lines on the phone screen). He submitted the phone, and upon inspection, I discovered that the phone is indeed damaged. I now need to write a letter to the higher authority to inform them about the damaged phone. Please advise me on how to draft a letter regarding this matter.

Dear Pratik, While talking over the phone, did you receive a letter from the recipient stating that it was handed over in damaged condition? If you have obtained this letter, all you need to do is send it to the authorities along with your covering letter.
