As the work is normally carried out by an unskilled workforce, effective supervision must be planned and executed to avoid such incidents/accidents in the workplace. I don't think the corrective or preventive measures you provided are appropriate. There is a need to think in another manner. The problem was not addressed properly; the hazard is still not resolved. If you have conducted a risk assessment of the activity, please share it because there are many things to discuss.

What you have mentioned are secondary things; the investigation is incomplete.

I have shared the incident report that I obtained from somewhere; hence, I do not have a detailed risk assessment report.
